Abstract

PROBLEM: tibial plateau fractures are articular and involve the proximal metaphysis of the tibia, although there may be traces that reach the diaphysis. These injuries have increased in frequency in recent years in Guatemala, possibly linked to traffic accidents, with the highest incidence being motorcycle accidents. OBJECTIVE: to determine the concordance by means of the weighted Kappa index between tomography and radiography according to the Schatzker classification in patients admitted to the lower limb service at the General Hospital for Accidents. MATERIALS AND METHODS: a retrospective descriptive study was carried out on the tomographic classification of Schatzker knee fractures and its comparison with radiography in patients of the general Accident Hospital of the Guatemalan Social Security Institute in the period from January to December 2020. RESULTS: The male gender was the most affected with 91% of the patients. The most frequent type of fracture of the tibial plateaus in tomography was type II with 32% of the patients. The concordance between the radiography and the tomography was 0.75 according to the Kappa index, which is interpreted as good concordance between the two studies. The most used treatment in patients was fixation with plate and screws. CONCLUSION: it is recommended to implement programs aimed at the population for the prevention of traffic accidents, raising awareness regarding the sequelae of this type of fractures and the repercussions both in the personal and work fields.

In the process of graduating in the Master of Medical Sciences with a Specialty in Radiology and Diagnostic Imaging at the University of San Carlos of Guatemala and the Guatemalan Social Security Institute of Guatemala. He has a postgraduate degree in Research Specialization from the CUNORI-USAC University Center of the East. She is a Doctor and Surgeon graduated from CUNORI-USAC.
